Our Policies

PRE SCREEN INFORMATION

  • All prospective patients are required to complete the pre screen questionnaire, this
    allows us to learn more about your needs and ensure that Dr. Ampiah can provide
    the care you seek in the outpatient setting.
  • After submission, you will be contacted within 24-48 business hours to
    either:
    •  Schedule an initial consultation with Dr. Ampiah or be provided with
      recommendations for other resources.

TELE PSYCHIATRY POLICIES

  • Patients must be in Florida at time of all telemedicine appointments. 
  • Due to federal and state laws, stable patients prescribed controlled substances must be seen once every 3 months. 
  • All patients on controlled substances require an in-person appointment at least once per year. 
  • New patients also require an in-office appointment prior to controlled substances being prescribed (*subject to change based on Ryan Haight Law).

ACCEPTED FORMS OF PAYMENT

  • Paramount Wellness Collaborative is an out of network, direct pay, private psychiatric practice. This means we are unable to accept any insurance plans. 
  • Accepted payments include all major credit/debit and HSA/FSA cards.
  • A valid card is required to be kept on file at the time of booking a new patient appointment and to continue treatment. Payment details are not retained by our practice, it is securely and confidentially vaulted through the Paramount Wellness Collaborative’s integrated payment platform on the patient portal.
  • A 50% deposit is required at the time of booking all new patient appointments, this is credited toward the total appointment fee, due at time of appointment. Follow Up appointments will be charged at the beginning of the appointment. 
  • Due to federal and state law, Dr. Ampiah is unable to see patients with Medicare/Medicaid at this practice, even if the patient wishes to pay out of pocket.  
 
  • Upon request, an out-of-network ‘Superbill’ will be available after the appointment which can be used to request reimbursement from your insurance provider. 
  • To ensure that your insurance plan offers such benefits, patients can inquire regarding coverage of the following CPT codes often used in documentation: 


– Initial Evaluations: 99204, 99205, 90833, 90836, 90838, 90792
– Follow-Ups: 99213, 99214, 99215, 90833, 90836, 90785, 99417
– Phone-Consults: 99441, 99442, 99443.

Rates

  • Initial Consultation Virtual/ In Person: $480 (80 minutes)
  • Follow up Appointments Virtual/ In Person: 
  • Stable Medication Management Follow Up: $220 (30 minutes)
  • Extended Follow Up visits may be required due to crisis or any additional needs patient or family may have. If 35-45 mins duration, $290; if 50-60 mins follow up required, $360 vs $440.

  • Fees are subject to change and patient will be notified in advance. 
  • Accepted payments include all major credit/debit and HSA/FSA cards.
  • An active card is required to be kept on file at the time of booking a new patient appointment and to continue treatment at Paramount Wellness Collaborative, Inc. This card will be pre-authorized in advance and then charged PRIOR to the start of appointment. If appointment exceeds allotted time, the excess fee will be charged after the appointment. Should the prior authorization be declined, the appointment will be held until full payment received. 
  • PWC Inc. does not retain patient payment details, it is securely and confidentially vaulted by our payment processing department.  

attendance policies

  • The scheduled appointment time is reserved specifically for each patient, thus if you are unable to attend the appointment, please cancel at least 2 full days (48 hours EXCLUDING holidays) prior to appointment. Cancellations are accepted by phone, patient portal or via automated reminder text message which is sent 3 days prior to each appointment. 
  •  There is a 10 minute grace period for tardies, please call or send a message via the portal if you are delayed. 
  • There is a 50% charge of scheduled appointment-type in case of late cancellations which are those cancelled within 24-48 hours of the appointment. 
  • If appointment is cancelled within 24 hours or patient does not show for the appointment, the FULL appointment rate will be charged. *Dr. Ampiah understands that life happens and so offers a one-time waiver if a follow up appointment is missed due to a no show or late cancellation.
  • If there are 3 or more no shows or late cancellations in a 12-month period, patients may be discharged from the practice due to violation of the office attendance policy.

COMMUNICATION/ REFILL/ PORTAL REQUESTS

  • It is customary for refills to be sent at regularly scheduled appointments.  Dr. Ampiah will provide refills to last until the next appointment as discussed at the time of the visit. 
  • Should there be a need for a refill in between visits, a $25 refill fee will be charged. Controlled substances can only be dispensed once every 30 days and thus may not be eligible for additional refills. 
  • Refill requests are to be sent through patient portal and are completed within 24 hours after payment received.   

COMMUNICATION 

  • Due to Paramount Wellness Collaborative being a small, weekend only, solo private practice, Dr. Ampiah may not always be available for immediate access. Thus, the preferred mode of communication is via the Charm Health patient portal as it is HIPAA compliant and secure. 
  • If you desire to call the office and we are unable to answer, please leave a voicemail.
  • Messages (voicemail and portal) are checked daily.
  • Please allow up to 48 hours for Dr. Ampiah to respond to any messages (portal or voicemails).
 

Portal Etiquette: 

  • The phone line and patient portal are NOT for emergencies or acute crises.
  • Through the portal, you may ask questions about appointments, medications, side effects, and request refills in the portal as well as view your visit summaries and upcoming appointments.  If there is any in-depth portal communication regarding a particular concern, or if messages become too frequent or complex, then a follow up appointment will be scheduled.
  • The practice email should not be used for medical advice, prescription requests, or emergencies.

WHAT TO DO IN CRISIS/EMERGENCY

  • Dr. Ampiah understands that inherent to certain psychiatric conditions are risk of decompensation, while she will support her existing patients through any crisis, the nature of PWC being a solo private practice with limited office hours means she may not be available for immediate access. 
  • To ensure prompt care in any urgent crisis or emergency situation, if you require immediate assistance, are feeling unsafe, or are in an emergency or a crisis situation, please immediately call 911, or go to your nearest emergency room. 
  • Once patient is safe, then a call may be placed, or message may be sent through the patient portal so Dr. Ampiah is aware. She will contact patient (if they are not admitted) or, if escalated to a higher level of care, will collaborate as able with inpatient treatment team (once ROI is completed, if applicable).
